Lidocaine hydrochloride undergoes rapid metabolism in the liver, with both metabolites and unchanged drug being excreted by the kidneys. The biotransformation process includes oxidative N-dealkylation, ring hydroxylation, amide bond cleavage, and conjugation. N-dealkylation, the primary pathway, produces the metabolites monoethylglycinexylidide and glycinexylidide. These metabolites exhibit pharmacological and toxicological actions similar to Lidocaine hydrochloride, though they are less potent. Approximately 90% of administered Lidocaine hydrochloride is excreted as various metabolites, with less than 10% excreted unchanged. The main metabolite found in urine is a conjugate of 4-hydroxy-2,6-dimethylaniline.
