Description: Neostigmine methylsulfate,(m-hydroxyphenyl)trimethylammonium methylsulfatedimethylcarbamate or the dimethylcarbamic ester of 3-hydroxyphenyltrimethylammoniummethylsulfate (Prostigminmethylsulfate), is a bitter, odorless, white, crystalline powder.It is very soluble in water and soluble in alcohol. Solutionsare stable and can be sterilized by boiling. The compound istoo hygroscopic for use in a solid form and thus is alwaysused as an injection. Aqueous solutions are neutral to litmus.
It inhibits cholinesterase activity and causes ACh accumulation, thus exerting a cholinergic effect. It directly activates nicotinic receptors (N2 receptors) on the motor endplate of skeletal muscle. It has a strong excitatory effect on skeletal muscle. The gastrointestinal smooth muscle can promote gastric contraction and increase gastric acid secretion, and promote peristalsis of the small and large intestine, especially the colon, thereby preventing intestinal relaxation and promoting intestinal contents downward propulsion. The effect on gland, eye, cardiovascular and bronchial smooth muscle was weak. The effect on central nervous system is weak.
N-dimethylaminophenol and dimethylcarbamoyl chloride were esterified to obtain neostigmine, and then methyl bromide was used to form quaternary ammonium salt to produce bromoneostigmine. Neostigmine methyl sulfate is prepared by the reaction of neostigmine and dimethyl sulfate to quaternary ammonium salt.
